Blood Pressure and Heart Rate Log {Free Printable}

A few months ago my grandma’s heart doctor wanted her to record her blood pressure and heart rate multiple times throughout the day. I made a few edits to the log included with the Family Medical Binder and she loves it! Since it’s been working out so well for her, I thought you could use it too and/or share it with someone that you know.
